WPATH Assessments
Affirming hormone and surgery readiness assessments for adults pursuing gender-affirming care.
In-person WPATH (World Professional Association for Transgender Health) assessments in Halifax, or anywhere in Nova Scotia by secure video.
"I know who I am and what I want. I just need the letter."
You’ve already made the decision. You know what care you want and why. Then your doctor or surgical team tells you an assessment is required first, and suddenly there’s one more gate between you and the care you’ve waited for. You wonder if you’ll have to prove you’re “trans enough,” if your anxiety or your past will be held against you, or if you’ll spend months and money only to be told no.
Our WPATH assessments aren’t a test to pass. We take the time to understand your goals and your situation, so you get the letter your provider needs and a clear picture of what comes next.
Let’s start with a conversation.
WPATH Assessments | Halifax
Why Choose A WPATH Assessment?
A WPATH assessment is a readiness assessment for gender-affirming medical care, following the World Professional Association for Transgender Health Standards of Care. Your clinician meets with you to talk through your gender experience, your treatment goals, and your understanding of the procedure you’re seeking. The conversation also covers informed consent, any relevant physical or mental health concerns, fertility and reproductive considerations, and the supports you have or might need before and after treatment.
It isn’t a test of whether you’re “trans enough,” and we’re not looking for a particular story. There’s no one right way to transition. Therapy isn’t a requirement either. This is a collaborative, affirming process meant to support your decision and confirm you meet the criteria for the care you’ve chosen. When the criteria are met, your clinician at East Coast Psychotherapy & Trauma Clinic prepares the letter or documentation your provider requires.
The conversation also covers informed consent, your personal and family medical history (physical and mental), fertility and reproductive considerations, and the social and financial supports you have or might need before and after treatment.

A WPATH Assessment Can Support:
Hormone Readiness
For adults starting feminizing or masculinizing hormone therapy. One letter from a WPATH-trained mental health or medical professional is typically all your prescriber needs.
Surgery Readiness
For individuals pursuing gender-affirming surgery (sometimes called sex reassignment surgery). Most surgical teams require a support letter from a WPATH-trained mental health provider alongside a physician specialist letter.
MSI-Funded Surgery
Documentation to support your application for gender-affirming surgery coverage through Nova Scotia’s MSI program.
Informed Consent
A clear walk-through of the treatment you’re seeking, its benefits and risks, and what to expect, so your decision is a fully informed one.
Health Considerations
Any physical or mental health factors worth planning for. Having anxiety, depression, or a trauma history doesn’t rule you out.
Fertility & Reproductive Planning
Space to think through fertility preservation and reproductive options before treatment begins.
Support Before & After
A look at the practical, social, and emotional supports around you, and referrals if there are gaps.
Next Steps When Criteria Aren't Met Yet
If further assessment, support, or medical consultation is recommended first, your clinician explains why and helps you get there.

Your Identity Is Respected From the First Call
Your name, pronouns, and gender identity are affirmed throughout, from intake to your final letter.

Mental Health Doesn't Disqualify You
Anxiety, depression, trauma, or neurodivergence don’t stop you from accessing care. We look at what support might help alongside treatment, not at reasons to say no.

Questions About WPATH Assessments
What is a WPATH assessment?
A readiness assessment for gender-affirming hormones or surgery, following the World Professional Association for Transgender Health Standards of Care. It confirms you understand the treatment you’re seeking, can give informed consent, and meet the criteria, and it results in the letter your medical provider requires.
Do I have to prove I'm trans?
No. The WPATH assessment isn’t a test of your identity, and there’s no particular narrative we’re looking for. Most people who come to us already know who they are. Our job is to support your decision and provide the documentation you need.
Is this for someone who's still exploring their gender identity?
No. A WPATH assessment is for people who’ve already decided on a specific medical or surgical step and need documentation for it. If you’re still exploring, our therapists offer affirming support without any assessment attached.
Will anxiety, depression, or trauma stop me from getting a letter?
No. Mental health concerns don’t disqualify you from gender-affirming care. Where relevant, your WPATH clinician may talk with you about supports that could help alongside treatment.
How many appointments will it take?
WPATH doesn’t set a fixed number. Most people meet with their clinician for one or two sessions, and the process is shaped by the type of care you’re seeking, your circumstances, and what your provider needs. Your WPATH clinician will outline the expected timeline at your first appointment.
What does a WPATH assessment cost?
Do I need a referral?
Does the letter guarantee my treatment will be approved?
No. The WPATH assessment supports your access to care, but final decisions about treatment, surgery, or funding rest with your medical provider or MSI.
What if I don't meet the criteria yet?
Your clinician will explain why, what’s recommended first, and how to get there, whether that’s further assessment, support, or a medical consultation. The goal is to help you move forward rather than to close a door.
What do I get at the end?
When the criteria are met, an assessment letter or documentation for your physician, hormone prescriber, or surgical team, summarizing the assessment and confirming your eligibility and capacity to consent. Your WPATH therapist reviews it with you and answers any questions.
Still have questions? We're happy to answer them.

- A clearer understanding of the treatment, its benefits, and its risks
- Answers to practical, medical, and emotional questions they hadn't thought through yet
- A plan for fertility, supports, and recovery before treatment begins
- Confidence that their identity was respected through the whole process

When you're ready to move forward with gender-affirming care, we're ready to help you get there.
